Enrollment Documents

Required documents for students enrolling in K-12:

• Registration Form

• Original Birth Certificate (we will make a copy and return the original)

• Immunization Record

• Proof of Residency Affidavit with attached documentation(Utility Bill, Purchase/Lease Agreement, Property Tax Bill, etc.)

• MCIR Consent Form

• Request for School Records

Additional required documents for students enrolling in Kindergarten:

• Proof of Hearing and Vision Screening from family doctor or health department

• Kindergarten Oral Health Assessment Form

• Kindergarten Age Waiver (if applicable)

Additional required documents for students enrolling in Grades 9-12:

• Athlete Transfer Form
